In Ultima V: Warriors of Destiny, Captain Johne and three others were the only survivors when the Ararat sank.  He found a bag with three pieces of the [[UO:Gem of Immortality]] in it.  Those pieces drove him to kill his companions.  The three Shadowlords were born from their blood.
