This item is "Recovered from an Ambushed Trade Route". It comes as a rare drop from doing trade quests during the 20th Anniversary global event.
It is described as: "The amulet bears a large pentagram hanging from a series of interlocking beads. Small bits of daemon flesh grind between the links as you manipulate them."
